New patients (over the age of 5) will be offered a free health check.
The health check helps us to assess and review your health care needs and record clinical data such as height, weight and blood pressure. You will be asked about:
- Current and past illnesses
- Operations
- Illnesses that run in the family
- Medication and allergies, including repeat medications that may need to be added to your record
- Details of screening tests such as cervical smears
- Immunisations you’ve had
Any health issues/concerns to that they will be directed to the appropriate nurse/clinician if they have any health issues they wish to be discussed/reviews due
